The Secret Pandemic Drills: A Controversial Move?

Imagine a deadly virus, specifically targeting children, spreading across the globe from a remote island in Southeast Asia. This isn't a scene from a sci-fi movie, but a real-life pandemic simulation exercise, codenamed Exercise Pegasus, conducted across the UK this autumn.

Exercise Pegasus, the largest of its kind in the country's history, involved a fictional scenario where a novel enterovirus, "EV-D68", emerged and rapidly spread worldwide. Unlike COVID-19, this virus was most lethal to the young, causing respiratory failure, brain swelling, and even paralysis in infants, children, and teenagers. The consequences were dire: travel restrictions, school closures, and mask mandates in the UK and beyond.

But here's where it gets controversial: during the drill, ministers had to "wargame" dealing with protests over social distancing. This comes at a time when the Covid Inquiry's second module found the UK's early response to COVID-19 was inadequate, leading to a series of devastating national lockdowns. The inquiry highlighted the profound impact of school closures on children, bringing their ordinary childhood to a halt.

Exercise Pegasus was designed to simulate a realistic pandemic scenario, a first in nearly a decade. The real EV-D68, first isolated in California in 1962, has caused respiratory issues, meningitis, and a polio-like paralysis known as acute flaccid paralysis in children.

Sir Peter Horby, a professor at the University of Oxford, commented on the choice of an enterovirus, stating, "It's a good choice because it's a real possibility with real risks but is different from what we've seen before." He also questioned whether anything had changed since COVID-19, asking if the systems and responses were different.

In the first phase of the exercise, participants were informed of an outbreak on the fictional Island of Musiyana, which had already spread to Malaysia and Singapore. The outbreak followed a local food festival, and a child's death on September 17th was reported. The virus was considered to have pandemic potential.

Phase 2 saw the World Health Organization declare a pandemic, leading to school closures and increased pressure on hospitals. Protests over social distancing also emerged. COBRA was informed that, similar to COVID-19, the virus could spread through respiratory aerosols and droplets, and asymptomatic spread was a possibility.

Phase 3 brought a national lockdown, with non-essential businesses ordered to close. There were also concerns about potential food shortages if the virus, thought to be carried by pigs, infected UK herds.

The first part of the Covid Inquiry criticized the UK's pandemic plan, stating it focused on only one type of pandemic and failed to consider prevention or proportional response, neglecting the economic and social consequences.

It's unclear what measures ministers ordered during Exercise Pegasus to prevent the virus's spread to the UK, but the exercise aimed to test government decision-making around containment and mitigation.

The second part of the Covid Inquiry found that school closures during COVID-19 lockdowns had a profound impact on children, bringing their ordinary childhood to a halt. No plans were in place for a quick switch to remote learning.

In Exercise Pegasus, the imagined virus's lethality to children presented ministers with a unique challenge regarding schools.

It's believed that, unlike Exercise Cygnus in 2016, no ministers walked out during Exercise Pegasus. During Exercise Cygnus, then-Health Secretary Jeremy Hunt refused to continue when it became clear hospitals would be overrun and he was asked to decide who should receive care.

Trudie Lang, Professor of Global Health Research at the University of Oxford, emphasized the importance of pandemic drills like Exercise Pegasus, stating, "These exercises are really important as they throw up barriers and innovations you might not have foreseen, but they should be made open and shared internationally in a curateable form."
